SECURA Insurance | 2017 Associate Benefits At SECURA, we don't just care about you as an associate. We care about you as a person. That's why we offer an á la carte benefits program. It allows you to select options that fit your needs. Health Benefit We provide a solid health insurance plan for associates. Those in our Appleton offices have a choice of a Health Maintenance Organization (HMO) plan or a Point-Of-Service (POS) plan offered through Network Health Plan. Associates in our Middleton office and the field may choose between two plans through Cypress Benefits' Preferred Provider Organization (PPO) Program. We award additional benefit credits to associates who: • Waive health benefits (with proof of other coverage). • Participate in our annual Health Risk Assessment process. Dental Benefit Enjoy the benefit of a well-kept smile! We offer you a comprehensive dental plan with two different options through Delta Dental. A Dental Associates option also is available to home office associates. We provide coverage for preventive, corrective, major, and orthodontia-related services. Life Insurance Our basic term life insurance benefit is $50,000. You also receive an accidental death and dismemberment benefit equal to the amount of your basic insurance benefit. You may choose to enhance your life insurance coverage by purchasing additional term life insurance and dependent term life insurance through payroll deduction at group rates. Long-Term Care Insurance You can purchase long-term care insurance for care received at home or a facility in the event that you need help with daily living activities or suffer from severe cognitive impairment. Choose from three different plans. Specified family members also may purchase this coverage through SECURA at group rates. Vision Insurance You can purchase a vision plan that provides you and your family with vision care services on a regular basis. Choose from two different plan options through Superior Vision. Time Off With Pay At SECURA, we know your free time is valuable. We also recognize taking a break to do what you enjoy is beneficial for your overall wellness. That's why we offer generous programs. Vacation Year of hire is prorated based on hire date. This could be volunteering, treating yourself to a day at the spa, or enjoying the outdoors... anything that helps you relax and rejuvenate. This day is built into your vacation balance. Short-Term Disability/Sickness This plan provides you with replacement income for time not worked due to sickness or nonoccupational bodily injury. Replacement income is equal to the amount specified in the schedule of coverage. Coverage continues for as long as you are medically unable to work or until 180 days have passed since the injury/sickness, whichever comes first. SECURA pays for this benefit.
